Proof of residency
Every family in the district will be required to complete an Affidavit of Residency form as well as presenting documentation to support your residency status before your student can be enrolled. If you are unable to complete one of the affidavits below please contact Central Office to schedule an appointment to begin the enrollment process.
Below are forms and information to assist you in this process. Please choose the applicable form, complete and email it to residencyenrollment@lonejackc6.net. You MUST fill out one of the affidavit forms and send with your proof of residency.
If you are unable to email proof of residency forms. Please print and bring necessary documentation to enrollment.

High school new student enrollment: **If you know a new family who has not attended LJC6 prior to the 2023-24 school year, they will work through a: NEW STUDENT USER EMAIL process. Please click on the link below:
