Proof of residency

Every family in the district will be required to complete and upload an Affidavit of Residency form as well as presenting documentation to support your residency status before your student can attend classes. If you are unable to complete one of the affidavits below please contact Central Office to schedule an appointment to begin the enrollment process.

Below are forms and information to assist you in the residency verification process. Please choose the applicable form, complete and submit in the residency verification section of your online enrollment form.  You MUST fill out one of the affidavit forms and upload with your proof of residency document.

Back to school online registration for ALL students grades K-12 

For the 2025-2026 school year, Lone Jack C6 Schools will utilize online registration. If you are unable to complete all steps in the google form, please note dates you can return proof of residency documentation and/or get assistance by district personnel.

At Lone Jack Middle and High School, Monday, August 12th 2:00 - 6:00 pm.

At Lone Jack Elementary, Tuesday, August 12th 4:00 - 7:00 pm at the Back to School Bash!

STEP 1

You will need to complete one of the following forms online. PLEASE COMPLETE A FORM FOR EACH CHILD. (If you have 2+ children, please fill out a form for each child.)

STEP 2

In Infinite Campus -- ALL students must have an updated Parent Portal for registration to be complete.

• Log into your Infinite Campus Parent Portal

• Go to MORE (left side of screen)

• Online Registration (begin or update)
