What to do before you come for Round Up Night:
ALL NEW STUDENTS need to be registered in Infinite Campus. Below is the link to complete this process.

Copy of birth certificate
Immunization record
Applicable completed affidavit of residency form from the list above with appropriate documentation.

Kindergarten Screening
Information will be available at Kindergarten Round Up on April 15, 5:00-6:00 PM at Lone Jack Elementary.
Families will have the opportunity to sign up for Kindergarten Screening at our “Kindergarten Round Up” night on April 15th from 5:00p.m. – 6:00p.m. If you are unable to attend the "Kindergarten Round-Up" event, you may contact Kristi Glover by phone at 816-697-3539, menu option (*) or Email kglover@lonejackc6.net to schedule a screening time.
Proof of residency
Every family in the district will be required to complete an Affidavit of Residency form as well as presenting documentation to support your residency status before your student can be enrolled. If you are unable to complete one of the affidavits below please contact Central Office to schedule an appointment to begin the enrollment process.
Below are forms and information to assist you in this process. Please choose the applicable form, complete and email it to residencyenrollment@lonejackc6.net. You MUST fill out one of the affidavit forms and send with your proof of residency.

If you are unable to email proof of residency forms. Please print and bring necessary documentation to enrollment.
Back to school registration for ALL students grades K-12
For the 2023-2024 School Year, Lone Jack Elementary will be implementing a $25 Instructional fee for every student to be paid at enrollment.
You will also need to complete the following forms online.
School Bus Transportation Rules and Regulations (required for all students regardless if they drive/walk to school)
Take Home Policy Google form (4-5 grade only)
Network Acceptable User Policy Google form (1st-5th grade)
Elementary new student enrollment
For the 2023-2024 School Year, Lone Jack Elementary will be implementing a $25 Instructional fee for every student to be paid at enrollment.
New students will need to provide copies of birth certificate, immunization record and applicable completed affidavit of residency form from the list above with appropriate documentation. The following online forms need to be filled out:
